Fees & Info
Dr. Bulfamante is an out-of-network provider and clients may seek reimbursement from insurance carriers. Clients are provided with a monthly superbill to submit to their insurance for full/partial reimbursement, dependent on their insurance provider.
Payment is due at the time of the service. Individuals are recommended to contact their insurance provider to inquire about out of network benefits. Below are some questions you may want to ask:
Does my plan provide out of network benefits for behavioral health care with a licensed psychologist?
What are my out of network benefits for a psychological evaluation?
Is there a deductible to be met before I am reimbursed? If so, what is it and how much have I met?
What percentage of the fees are covered?
Is there a session limit? If so, what is the time period?
Cash, check, and all major credit cards are accepted for payment. Dr. Bulfamante requires at least 24 hour notice for cancellations. Full fees will be charged for late cancellations or no shows.
